OPP screening - Vérification policière

OPP criminal checks are mandatory for all team representatives.   Both forms ( OPP criminal check and HMHA criminal check ) are available on our web site, follow to the "forms" menu.   You will require the OPP & HMHA  forms signed by the Association's president, paperwork needs to be handed to Luc Bélanger - La vérification policière est obligatoire pour tous les représentantes d'équipes.  Vous trouverez les formulaires sur notre site web, suivre au menu " forms ".   Vous aurez besion de faire signer les deux formulaires ( OPP criminal check et HMHA criminal check ) par votre président d'Association.  La documentation doit être acheminé à Luc Bélanger
